Overview

Log notes represent the working history of the person’s life as it pertains to the time period they receive services from DSPD.

Log notes can be created by DSPD employees, Private Support Coordinators or they may be system generated. Log notes should contain information about the person's welfare, status and condition.  They should be clear and concise, containing enough information for workers to quickly glean information to assist the consumer in any situation.
