Overview
Log notes represent the working history of the person’s life as it pertains to the time period they receive services from DSPD.
The following list are types of notes to include that document everything related to the person’s life:
It should include information/monthly status reports about the services they receive
The person’s achievements
The progress they are making on their personal goals
The status/condition of their Person Centered Service Plan along with monthly reports regarding its implementation
The monitoring of the challenges (physical or otherwise) he or she faces
The documentation of and follow up about the health and safety problems they encounter, etc.

Any DSPD employee who is involved with the person’s case can enter a log note about the work they have done for or in behalf of the person. The person’s Private Support Coordinator is expected to be the primary author of monthly log notes regarding the person’s welfare, status and condition. The log note’s author has 45 days from the date the note is created in the system to finish it. After the 45 day window has expired, the note’s original text cannot be changed. The note, however, can be amended by writing a separate note in the amendment text box.
Step 1: Go to the Main menu and click the Log tab:

This will take you to the Consumer Log screen.

Step 2: Click on
to create a new log note.

Written Correspondence

Do not copy/paste text from a source like MS Word into USTEPS. When you highlight a block of text and copy it, the block includes formatting characters that the USTEPS database does not recognize. This will display as garbage text in the log. You may first create or copy the text into a simple text editor, e.g. Notepad, and copy from there or enter the log note directly into USTEPS.
Creating Log Notes for Consumers not on a worker's case load:
Log notes can be created by any USTEPS user on any USTEPS consumer. If a note is created by someone other than the caseworker, the caseworker will receive a system generated message telling them that a log note has been created for the named consumer, along with the name of the person creating the log note.
