Incident Reporting

Incident reports are about the people served by the provider.  As a result, the report focuses on one person - the person in services.  The incident report data can include information about other people and things involved in the incident, such as witnesses, damaged property, provider staff, other victims, perpetrators, etc.  Even so, the report must focus on the person and their situation in relation to the incident.

 

 Reportable Incidents:

An incident should be substantial enough to warrant reporting to DSPD.  That is, not all minor issues that happen in the person's life rise to the level where a report must be issued.  Guidance about which situations and circumstances require an incident report are below.
