Incident Reports Guidelines

 

An Incident Report is created in and submitted to UPI whenever a reportable Incident happens to a person receiving services through DSPD.

Some incidents involve significant, important issues while others do not.  DSPD does not have to be notified about every minor issue that happens in the person's life.  Likewise, some incidents may involve circumstances that are important for the provider to document but are still not significant enough to report to DSPD.

If the provider is planning to submit a report, they must submit the incident notification within 24 hours of its discovery.  Then, the provider must submit the report within 5 business days of its discovery.

Both providers and Support Coordinators can enter and submit incident notifications and reports directly in UPI. The party who first learns about the incident should create and submit the report.  In the case of an incident involving a person in services utilizing the Self-administered service model the Support Coordinator is responsible for submitting the Incident Report.
