PCSP Pre-Planning
Several steps must be done as pre-steps to building Person Centered Support Plan.
1. Eligibility Review: Once the person is enrolled in the waiver, their eligibility must be reviewed annual in order to ensure they remain eligible for the waiver's requirements and services. DSPD staff are responsible for reviewing and recording the person's annual eligibility decision.
2. Review the Supports Intensity Scale (SIS) and the "TO's" and "FOR's": The SIS must be redone at least every five years. In addition to fulfilling waiver requirements, the SIS helps identify items that are important "TO" and "FOR" the person. "TO" items represent issues, things, circumstances, etc., that give value and meaning to the person. They lay the foundation for the person's goals. "FOR" items represent issues, things, circumstances, etc., that are important to maintain the person's health and safety. They justify the activities and services DSPD pays contract providers to deliver to the person. The support coordinator is responsible for reviewing and classifying the "TO's" and "FOR's" in cooperation with the planning team.
3. Review the Person Centered Profile: As apart of the general review of the person's case file, the Person Centered Profile should be updated prior to the planning meeting.
4. The Annual Review of Goals, Supports and Services: In order to ensure the new goals, supports and services are adequate to serve the person's needs, the old goals, supports and services from the previous plan cycle need to be reviewed.
5. Preparing for the Planning Meeting: Before the content of the PCSP is finalized, the planning team should meet to review what occurred during the last cycle and what the person needs in the new one. Additional planning efforts include:
a. Designing the Agenda, preparing to manage an effective meeting, etc. (see Sample Agenda)
6. Planning Meeting Minutes: After the planning meeting is completed, the discussion / decisions made during it should be captured.
