The ‘Planning Meeting Minutes’ section of the PCSP is designed to be an area for input from the support coordinator regarding various items from the planning meeting itself. Some considerations that may be included in this section include; who attended the meeting, what was discussed in the meeting, what information was reviewed during the meeting, any decisions made during the meeting.
Other considerations may include documenting the review of the following:
· Human Rights2
· Signing self-administered service agreements2
· Reviewing medications2
· Reviewing provider records2
· Reviewing SIS or CBIA2
· Reviewing health and safety issues2
The overall purpose of the ‘Planning Meeting Minutes’ section of the PCSP is to include relevant items that are discussed for the person, that do not have a specifically dedicated area on the PCSP.
This section required by Support Coordination External contract, DHS Procurement No. 90514 and its amendments. Specific content of this section is not outlined in contract. The content is at the discretion of the author of the PCSP.
