When preparing to write Person Centered Plan, the SCE should note that there are different areas on the plan for them to put information about TO’s and FOR’s. Below is a simple explanation for each area.
Person Center Profile1
The section under the person centered profile, listed as “other consideration” will have a place for the SCE to list TO/FOR’s. This section is for the SCE to list goals important TO the individual but is not a current goals present at this time. These (items) goals can be listed as goals in the future or not applicable.
The FOR section is for the SCE to include any support and or services provided to the individual that are not funded by DSPD or are not directly related to the individuals goals. The support team identifies critical issues that are not already address as “personal goal related” supports. This can also be used to highlight health and safety issues.
Assessments1
The other area the SCE will find TO and FOR’s is under the assessment section. The assessments contribute toward the development of the PSCP and the identification of items most important “TO” and “FOR” the person.
The TO’s listed in this section must be linked to a current goal the individual is working towards. Remember TO’s are a person goals, passion and interests.
The FOR’s listed in this section are for non-goal support or current health and safety issues that need to be addressed in the Person Centered Plan.
Remember to always send out the TO and FOR list to the provider and other participating team members to prepare for your Person Centered Plan. The TO and FOR should be reviewed to ensure they accurately reflect the current goals the individual wants to work on.
