The Person Centered Support Plan cycle contains the framework within which the plan is formatted and structured.

 

If the plan is ongoing and the person expected to remain enrolled in services, then the plan should last a full 12 months or 365 days.

The plan cycle consists of the following critical dates.

1. The plan's Effective Start Date.

2. The plan's Effective End Date.

3. The plan's Signature Date.

4. The plan's Planning Meeting Date.

 

The span of the Effective Start and End Dates represents the plan's total duration.  For example, if the plan begin 5/1/2017, then it is expected to end on 4/30/2018 assuming no disruption exists in the person's life where they are removed from services.

The Effective Start Date cannot be set on a date that is less than (before) the plan's Signature Date.

The Plan's Signature Date represents the date when the person signed the plan (which means they agree with and accept the scope and scale of the services prescribed in the plan).  It authorizes the services prescribed in the plan to start on the Effective Start Date.  The Signature Date should not be more than 31 days before the plan's Effective Start Date (i.e. the person should not have signed the plan more than 31 days before the is scheduled to start).

The Planning Meeting Date is the date when the planning meeting was held.
