Closing the Plan Early Because the Person left Services
Since no one is required by law to be served by DSPD (unless they are court ordered to do so), the person can choose to voluntarily or involuntarily leave services.
If the Person Centered Support Plan has to be closed early, the following steps specify how it is done.
1. All of the services on the PCSP Budget that have an end date after the date the plan is expected to close must be changed (set equal) to that end date.
2. The units on each budget line must be checked / changed such that they match what the provider did / is expected to spend through the date the plan is expected to close.
3. The End Date of each support must be changed (set equal to or before) the date the plan is expected to close.
4. The plan must be "Activated" at this point in the process. All of the changes made to the services on the budget must be sent to CAPS (the Department of Human Services payment system) before the plan is finely closed. This step gives the support coordinator the opportunity to review the data before they take the final step of closing the plan.
5. The plan's Effective End Date must be changed (set equal) to that end date.
After the plan's Effective End Date is changed and saved, the PCSP will be set to close on that date. If the date is in the future, the plan will remain open and active until that date arrives. However, if the date is in the past, then the plan will automatically, instantly close as soon as the change is saved.
