Incidents Types That Must Be Reported

 

The following is a list of incident types that must be reported to DPSD via UPI.  The list is not all inclusive.  Some incident types not included below may still need to be reported because of their unusual nature or circumstances.

  1. Missing persons who have been missing for at least 2 hours

  2. Injuries requiring medical treatment, including self injury

  3. Hospitalizations due to medical or physiatric reasons

  4. Property destruction when the damage is over $500.00

  5. Drug and/or alcohol abuse

  6. Law enforcement involvement

  7. Suicide threats, and/or attempts

  8. Aspiration or chocking that required intervention

  9. Emergency Behavior Interventions

  10. Behavior Interventions listed in the persons Behavior Support Plan

  11. Seizures or seizure like activity when the person does not have a seizure diagnoses

  12. Alleged or substantiated abuse, neglect or exploitation

  13. Medication errors if the person experienced adverse side effects from the error

  14. Emergency Room visits

  15. Fatalities
