Intake Overview

 

The Intake Process is designed to gather data regarding the person’s background, family circumstances, medical condition (s), problems, diagnosis (es), functional limitations, etc., in order to understand their needs and render an appropriate eligibility decision. The process is time limited and governed by rules that specify the person’s obligation to deliver the minimum amount of information for the Division to establish an eligibility decision.

The purpose of Intake is to determine a Person's State Eligibility to receive DSPD services.  Those found eligible are added to a managed list of consumers waiting for services.  Intake Processing ends for a consumer when the"eligibility" decision has been made.  There are three processes within Intake Processing; Initial Contact Decision, Intake Data Collection, and Intake Decision (Eligibility).

NOTE: Once the person is found eligible and waiting for services, all services rendered must be paid in CAPS.

Intake Overview:

Data is initially collected in order to be able to assess a Person's eligibility for DSPD services.  Intake is routinely updated to insure that we can contact a consumer, and the updated data is also used to periodically re-assess a Person's eligibility for specific services.

Step 1 - Start Intake Status:  When the Form 1-1 is returned to DSPD with the required data, the worker begins the 90-Day Data Collection Period. The initiation of the intake begins with obtaining the person’s PID.

Step 2 - Collect Data: The worker asks the person to provide a complete social history, diagnosis information, third party assessments, etc. They also meet with the person to administer an appropriate assessment (e.g. the ICAP for ID/RC, the CBIA for ABI or the MDSHC for PD).

Step 3 - Record the Eligibility Decision: An eligibility decision can be recorded anytime during the ”90-Day Data Collection Period” if the worker has enough information to do so.
