Service Code Criteria and Documentation

Each service code available in the Request for Services process has corresponding criteria.  Some services have several criteria.  Others have a few.  Each criterion is designed to clarify the scope and purpose for which the service should be used.  

Documentation recommendations and requirements accompany all criteria.  Any document can be used to address a criterion.  Even so, the worker should be mindful about which document(s) they choose to attached to the criterion.  The document should be current and relevant to the person's situation.
