Service Code Categories and Classifications

Each service code DSPD administers is organized by a set of categories and classifications.

The categories and classifications are intended to show which services have a similar scope and utility.  Each service code also comes with its own description designed to clarify the circumstances under which it can be used.

The scope and purpose of some service codes are very similar to those of other codes.  Consequently, when the support coordinator is looking for a service code to address the person's need(s), they expected to adequately research the options and select the code best suited for the situation.  

Some service codes are not available for use in some waivers.  As a result, the support coordinator should also consider the code's availability in the waiver the person is served by.

Description

Behavior Behavior Consultation Services BC1 The targeted behavior may be emerging, annoying, worrisome, objectionable, singular but not dangerous, and may interfere with learning or social relationships. The behaviors of the person shall not constitute an impending crisis.
Behavior Behavior Consultation Services BC2 The targeted behavior(s) is serious but not potentially life threatening behavior problems that may be complicated by medical or other factors.
Behavior Behavior Consultation Services BC3 The targeted behavior(s) is the most involved and complex, difficult, dangerous, potentially life threatening and resistant to change problems. Persons at this level of service have failed intervention, are frightening to others, and are severely limited to activities and opportunities they could otherwise access.
Budget Assistance Personal Budget Assistance Services PBA Services provides for support with financial matters, fiscal record keeping, and fiscal interaction on behalf of the Person.
Day Supports Day Supports Group DSG Day services are intended for Persons who need more supervision and support developing work and community living skills than a person who is enrolled in Supported Employment Services.
Day Supports Day Supports Individual DSI Day services are intended for Persons who receive supports during the day to avoid becoming isolated and to participate in the community. DSI is to be utilizied if the person has no access to DSG in the community or requires one-to-one supports. 
Day Supports Day Supports Partial DSP Day services partial provides a community habilitation program where people can gather in groups during the day after normally occurring school or training activities have ended for the day or week to avoid becoming isolated and participate in and contribute to their community.
Family Training Family Training and Prep Services TF1 Services provides for Person in SAS with the acquisition of skills necessary to function effectively as employers of their self-administered supports.
Family Training Family Training and Prep Services TFA Intended to help Person’s with the acquisition of skills necessary to function effectively as employers of their self-administered supports.
Family Training Family Training and Prep Services TFB Is intended for families with considerably more complex or dysfunctional issues than those receiving TFA services. Families receiving this service have been assessed as requiring a more sophisticated level of training and assistance than those receiving routine TFA services
Fiscal Administration Fiscal Management Services FMA

Fiscal Administration Fiscal Management Services FMS Services are provided to Persons and their families to hire, train, and supervise employees to provide direct services.
In Home Chore Service CH1 Chore services helps to maintain a clean, sanitary, and safe living environment for Persons that are unable to complete chore services on their own.  The service includes heavy household chores and is not a skills training service.
In Home Chore Service CHA Chore services helps to maintain a clean, sanitary and safe living environment for Persons that are unable to complete chore services on their own.  The service includes heavy household chores and is not a skills training service.
In Home Companion Services CO1 Companion CO1 provides care, supervision, and socialization services for a Person in the SAS model. Companions may assist or supervise the Person with such tasks as meal preparation, laundry, and shopping tasks but do not perform these services as discrete services.
In Home Companion Services COM Companions may assist or supervise the Person with such tasks as meal preparation, laundry, and shopping tasks but do not perform these services as discrete services. 
In Home Homemaker Services HS1 Services include general household activities, when the individual regularly responsible for those activities is temporarily absent or if the person is unable to manage the home and care for him or herself or other in the home. SAS Model 
In Home Homemaker Services HSQ Services provide services such as meal preparation and routine household care provided by a trained homemaker, when the individual regularly responsible for those activities is temporarily absent or if the Person or other resident in the home us unable to manage the home and care for him/herself.
In Home Overtime In-Home Services CH1 Chore services helps to maintain a clean, sanitary, and safe living environment for Persons that are unable to complete chore services on their own.  The service includes heavy household chores and is not a skills training service.
In Home Overtime In-Home Services CO1 Companion CO1 provides care, supervision, and socialization services for a Person in the SAS model. In Home Personal Assistance Services PAC Specific to the needs of a medically stable Person who is capable of directing his/her own care or has a surrogate available to direct the Person’s care.
Massage Therapy Specialized Supports Massage Therapy Services SSM Provides services for Persons needing massage therapy services.
Medical Personal Emergency Response Services PE1 Service provides for the installation of a medication dispenser box.
Medical Personal Emergency Response Services PE2 Service provides for monitoring of the medication dispenser box.
Medical Personal Emergency Response Services PE3 Service provides for the installation of personal emergency response devices and medication dispensers.
Medical Personal Emergency Response Services PE4 Services provides for the monitoring fee for PER and medication dispenser combo.

Medical Professional Medication Monitoring Services PM1 Service provides for monitoring by a Licensed Practical Nurse for one-to-one medication monitoring, laboratory testing, and services necessary to provide medication management to assure the health and welfare of the Person on a per session basis.
Medical Professional Medication Monitoring Services PM2 Service provides for monitoring by a Registered Nurse for one-to-one medication monitoring, laboratory testing, and services necessary to provide medication management to assure the health and welfare of the person on a per session basis.
One Time Purchases Environmental Adaptation Services EA1 Services are provided for Persons who have a need for specialized physical adaptations in their home as prescribed by a physician or other prescribing professional. The total cost of the adaptation cannot exceed $10,000.
One Time Purchases Environmental Adaptation Services EA2 Services are provided for Persons who have a need for specialized physical adaptations in their vehicle as prescribed by a physician or other prescribing professional. The total cost of the adaptation cannot exceed $10,000.

Residential Extended Living Service ELS Services are on a flexible basis to accommodate the Person’s needs, such as time between the school day and when residential services begin if time between school and home fluctuates regularly.
Residential Host Home Support Service HHS Provides a home like setting with a family that offers support, supervision, training, and assistance for adults in a certified residential setting or other certified private home.
Residential Professional Parent Support Services PPS Service [rovides a home like setting for children including habilitation, supervision, training and assistance in a certified private home or other certified residential setting. The following services are included as part of this code and can not be reimbursed separately (i.e. Chore, Companion, Homemaker, Personal Assistance, and Routine, Non- Medical Transportation).
Residential Residential Habilitative Services RHS Service includes maintenance of a Person’s health and safety; and assistance with activities of daily living, such as eating, bathing, and dressing for Persons who have a goal for community living who live alone or with roommates. 
Respite Overtime Respite Services RP1 Respite care to give relief to, or during the absence of, the normal caregiver. Does not include payment for room and board and is provided in the person’s residence or other approved residential setting. SAS Model 
Respite Overtime Respite Services RP6 Provides daily and overnight care to give relief to, or during the absence of, the Persons normal caregiver.  SAS Model with Room and Board 
Respite Overtime Respite Services RP7 Provides respite care for up to three individuals to give relief to, or during the absence of, the person's normal caregiver. SAS Model 
Respite Overtime Respite Services RP8 Provides routine respite care for up to three individuals as well as overnight care for the group in order to give relief to, or during the absence of, the person's normal caregiver. SAS Model 
Respite Respite Services RP1 Respite care to give relief to, or during the absence of, the normal caregiver. Does not include payment for room and board and is provided in the person’s residence or other approved residential setting. SAS Model

Respite Respite Services RP5 Provides overnight care to give relief to, or during the absence of, the person's normal caregiver, when they have exception medical and/or behavioral needs.  Exceptional Care with Room and board included. 
Respite Respite Services RP6 Provides daily and overnight care to give relief to, or during the absence of, the Persons normal caregiver.  SAS Model with Room and Board 
Respite Respite Services RP7 Provides respite care for up to three individuals to give relief to, or during the absence of, the person's normal caregiver. SAS Model 
Respite Respite Services RP8 Provides routine respite care for up to three individuals as well as overnight care for the group in order to give relief to, or during the absence of, the person's normal caregiver. SAS Model 
Respite Respite Services RPS Provides care rendered as a session to relieve, or during the absence of, the Persons normal caregiver. Session based. 
Start Up Costs Residential Start Up Costs STC Provides funds for activities that will be of time-limited duration or for start up costs of new programs.
Supported Employment Supported Employment Assessment SEI Service provides helps establish, maintain, and advance in competitive employment in integrated work settings. Persons are compensated at a wage commensurate with their level of training and development.
Supported Employment Supported Employment Co-Worker SEC Service supports Co- Worker who supports the Person in their efforts to obtain, maintain, and advance in competitive employment in integrated work settings. 
Supported Employment Supported Employment Day SED For Persons in work groups made up between two to eight Persons in their efforts to obtain, maintain, and advance in competitive employment in integrated work settings. 
Supported Employment Supported Employment Enterprise SEE Persons who are working competitively to establish, maintain, and advance their self-employment in business enterprises of the Person’s creation.
Supported Employment Supported Employment Individual SEI Service provides helps establish, maintain, and advance in competitive employment in integrated work settings. Persons are compensated at a wage commensurate with their level of training and development.
Supported Living Overtime Supported Living Services SL1 Reduced version of SLH and is used when parents or other relatives take the primary responsibility for the Person’s health and safety, management of benefits, medication observation and recording, and activities of daily life. SAS Model 
Supported Living Supported Living Services SL1 Reduced version of SLH and is used when parents or other relatives take the primary responsibility for the Person’s health and safety, management of benefits, medication observation and recording, and activities of daily life. SAS Model 
Supported Living Supported Living Services SLH For Persons who live alone in their own homes, with roommates, or a spouse or for adults who live with their parents. Activities are prioritized based upon the person’s assessed needs but always include maintenance of the person’s health and safety, personal care services, homemaker, chore, attendant care, medication observation and recording, advocacy, communication, and assistance with daily living.
Supported Living Supported Living Services SLN Reduced version of SLH and is used when parents or other relatives take the primary responsibility for the Person’s health and safety, management of benefits, medication observation and recording, and activities of daily life.
Transportation Motor Transportation Payment Service MTP Provides transportation from a Person's home or living facility to community habilitation programs or facilities that provide day supports.
Transportation Transportation Services DTP Provides transportation for a Person to gain access to waiver and other community services, activitie,s and resources specified by the Person Centered Support Plan. 
Transportation Transportation Services UTA Service provided independently as abus pass when transportation is not otherwise available as an element of another service. 
Transportation Transportation Services UTD Service provides for the utilization of a UTA route deviation fixed bus routes to gain access to Flextrans paratransit. 
Transportation Transportation Services UTF Combination of UTA and UTD. This code combines both rates into one.
