(PPS/HHS) Professional Parent / Host Home Worksheet: Community Supports

Use this worksheet to develop a daily rate for Professional Parent / Host Home services. 

1. Direct Support Costs:  Hours are usually entered as Hrs/Month.  PPS/HHS has a minimum of 12 monthly hours.  These are the hours per month for direct care staff to come into the home to train and assist the professional parent employee.

2. Staff Ratios:  Generally a 1:1 staffing ratio unless there are two individuals in the home who are receiving this service; it can be 1:2.

3. Other Supports:  This is the total amount of the professional parent stipend, respite, and other hours, etc.  You need to confer with the provider to determine the accuracy of this amount.
