Instructions for completing the budget worksheet

 

(DSP Qtr Hr) After School Program: Community Supports; 6 hour Maximum / 10 hour Maximum quarter hour worksheet.

Use this worksheet to develop a quarter hour rate for an after school program service.  This worksheet is used for an after school program when DSP needs to be a quarter hour service with a 1:1 staffing ratio.  This worksheet will also calculate a quarter hour rate with various staffing ratios.

A:  6 hour maximum:  This section is used for after school programs during the school year; not to exceed 6 hours per day.

  1. Annual Days Allocated:  Enter the number of annual days needed/approved; not to exceed 240 days without prior approval.

  2. Hrs./Day:  Hours of service needed per day (total not to exceed 10 hours)

  3. Staffing ratio:  reflects how much staffing ratio is required (how many hours of one to one and/or sharing of staff) 

  4. Average Daily Hours: This number is calculated by the worksheet based upon the information entered (staffing hours and ratios). This number may be needed to enter into the related fields of budgetary information in USTEPS.

 

B:  10 hour maximum:  This section is used for an after school program available during the summer months when school is not in session or for times when the school or day program is closed for 5 consecutive days; not to exceed 10 hours per day.

  1. Annual Days Allocated:  Enter the number of annual days needed/approved; not to exceed 69 days.

  2. Hrs./Day:  Hours of service needed per day (total not to exceed 10 hours)

  3. Staffing ratio:  reflects how much staffing ratio is required (how many hours of one to one and/or sharing of staff) 

  4. Average Daily Hours: This number is calculated by the worksheet based upon the information entered (staffing hours and ratios). This number may be needed to enter into the related fields of budgetary information in USTEPS.
