Activate the "New, Pending" plan
The activation of the "New, Pending" plan is the first version of the plan cycle. It should contain all of the goals, supports and services the person needs to lead a self-determined life as well as be kept healthy and safe. Even so, some circumstances may require the content of the new plan to be incomplete. For example, a provider's new contract may still be in process when the new plan comes due or the person may be in the process of selecting a new provider to deliver a service. Such variances are understood and should not impede the new plan's activation as defined by the support coordinator's contract / policy requirements. That is, the data can be omitted from the plan (activated without it) until it becomes available.
New, updated information can be added to the plan after its initial activation is needed / required.
Before the new plan can be activated, the person is required to the sign the appropriate signature page belonging to the type of plan / waiver / eligibility the person has. It contains the necessary statements, agreements, etc., the person is expected to certify by signing the form.
The support coordinator is required to document the general issues, circumstances, conditions, etc., that existed when the plan was originally activated in the space provided.
