Create an Addendum to the Active Plan

The PCSP is meant to be a living document.  It is expected to evolve with the ongoing, real time changes that happen in the person's life.  Consequently, if new information is added to the plan or if existing information is changed in the plan, the data is versioned such that one version of the plan can be clearly distinguished between another.

When an addendum (i.e. a new version) is created, the person is not required to physically sign another signature page in order to activate the plan.  Instead, the support coordinator need only report:

1. Who they spoke to about authorizing the change.

2. The date the conversation took place.

3. A description that explains the nature of the change, what change, why it changed, etc.  

 

The description should be clear and concise enough to help anyone who has no prior knowledge of the person / case understand what happened and why.
