Approve the Person's 1056
The Form 1056 can be accessed from the "Consumer" menu in UPI by doing the following:
1. Select the desired person.
2. Click on the "Approve 1056" link in the Consumer drop down menu

Click
the desired link or button
a. To finalize the approval process, click the "Approve 1056" button

b. To finalize the rejection process, write a comment about why the service code(s), amount and/or duration are inaccurate. Click the "Reject 1056" button

NOTE: a Form 1056 can be either approved or rejected. However, both decisions cannot be recorded simultaneously. Each form represents a change activated in the plan. The Form 1056 is required only when a "material" change is made to the plan- meaning the dates, rates, or units of service has changed.
